| Requesting Documents After Graduating |

| To request graduation certificates, academic transcripts, or other official documents after graduation, graduates may apply in person or via mail. |
 |
| Please Note: In cases where the application is made by a third party, a letter authored by the graduate and authorizing the third party to make the request must be provided. |
 |
| Official identification (e.g. driver's license, passport) is required to verify the identity of the graduate or authorized applicant. For requests made by mail, photocopies will be accepted. |
 |
| Requests made by telephone, fax, or email are not accepted.
There is no charge for issuing documents. |

| Application Methods |

Applying in person |
| Complete a "Certificate Issue Application Form" at the relevant office (listed below). The requested document(s) will be issued three business days from the date of submission. (10 business days are required for documents requested to be issued in English. ) |

Applying by mail |
| In an enveloped clearly marked "Request for Certificate" in red ink, please include the following items/information. The requested documents will usually be mailed within three business days of the receipt of the request. |

A completed "Certificate Issue Application Form" listing the following information (if an application form is unavailable, please clearly indicate the following information):

- Name
(certificates are issued bearing the graduate's name at the time of graduation)
- Date of birth
- Registered address (certificates are issued bearing the address of the student at the time of graduation)
- Current address and telephone number (please also include daytime contact information)
- Faculty, department/course, year/month of entrance, and year/month of graduation
- Reason for request and intended recipients of requested documents. (Please list one recipient for each copy requested)
- Types of certificates and number of copies requested
*To request that certificates be issued in English, please write "English Certificate," on your request and include the graduate's name in Roman script.

A self-addressed, stamped envelope (long format, No. 3: 235 × 120 mm). Domestic postage of 80 yen for one copy, 90 yen for two copies is required. Please inquire at the relevant university office for requests of more than two copies, or for requests to international destinations. |

Official identification which verifies the graduate's (and authorized applicant's) identity and current mailing address. |
